Drug & Alcohol Testing
Valid government-issued photo ID (driver’s license, passport, military ID, or state ID)
Any authorization form or testing order from your employer, court, or agency

Breath Alcohol Testing
Valid government-issued photo ID (driver’s license, passport, military ID, or state ID)
Employer or agency testing authorization form (if applicable)

DNA Testing
Valid government-issued photo ID for all adult participants
Birth certificate or ID for minors being tested
Any court or legal paperwork if testing is required for legal purposes

Fingerprinting (Live Scan & Ink)
Valid government-issued photo ID (driver’s license, passport, military ID, or state ID)
Fingerprinting form, ORI number, or agency instructions (if provided)
Payment (if not pre-paid by employer)
Notary & Remote Online Notary
Document(s) needing notarization (unsigned — you must sign in front of the notary)
Valid government-issued photo ID (driver’s license, passport, military ID, or state ID)
For Remote Online Notary: access to a computer or smartphone with internet and a webcam
I-9 Employment Eligibility Verification
Completed Section 1 of Form I-9 (employee’s responsibility)
Acceptable documents from the official I-9 list: • Option A: U.S. Passport (unexpired) • Option B + C: Driver’s license or state ID and Social Security card or birth certificate
Employer’s I-9 request or verification form (if applicable)
Insurance Adjusting Services
